Yes, you need a fill adapter. You would only fill to the scuba tank pressure, so your 4.5k would only get 3k of air at most. usually, you will only get a couple of full fills out of a scuba tank.

what about if its filled with Nitrogen instead of air? Is it more compactable? Home Depot should seel cheap 4500 psi compressors. Well i guess there would be the issue of you dying and all, but hey...

You can get scubas that hold above and beyond 3000 psi. The scuba shop guy said he could fill a standard 3000 psi one past 3k to about 3300 and still be ok. Im still skeptical about its safety but...he's the owner so...?
Just know that the tank pressure steadily decreases each fill eg., 2900 psi fill, 2750 psi, 2600 and so on. Those numbers arent exact but you get the point. Id say you can get like 10 or so fills above 2000 psi for a 47 or 68 c.i. tank.

The steel tanks can typically hold more i hear.

No difference between nitrogen and air, same thing for our purposes. I would not recomend filling your scuba tank over 3k unless you have a reg on it. The tank pressure is what you will be filling your nitro tank to...and while possible and fairly safe, I would not recomend overfilling your nitro tank even if it is only by a couple hundred psi...not worth the risk.
