Leaking HPA tank reg

I have a 3k psi tank with a PMI reg that i just recently got Hydro'd, i went and got it filled for the first time and 10 minutes later it started leaking out of the air input hole, and it was also leaking out of the allen key shaped hole that you use to adjust the reg with. This tank was working before i got it hydro'd but it is pretty old. Any advice before i take it to the shop?

Bad check valve, maybe. Try tightening the nipple itself. As for the allen wrench hole, I'm out.

New seal on the inpute quick disconnect and also a reg piston, but 2 orings failing at the the same time it wierd.
