What normally causes this? Being too aggressive with a pick to remove orings?

Yes. People should smooth the pointed end of their dental picks so they cant scratch. Go easy when removing orings with a pick.

Thanks everyone for all the help... I am probably going to just get a new valve and dump this one as-is at a discount. (Anyone wants to pick it up to fix it?)
