Intellifeed wire for warp

Does anyone have the instructions for making an intellifeed wire for the warp feed? I found the link on havoc's post that lead back to agd but the links are broken.

Here's the basics:
3/32 phone plug Radio Shack #274-289
22 gauge hook up wire, any color, RS# 278-1224
Roller switch RS# 275-017
Round head machine screws RS# 64-3010
Hex nuts RS# 64-3017
(Note: Screws are smaller than the holes so you can adjust the switch)

Wiring instructions:
On the switch use terminals 1 & 3 (The center and the one opposite the roller) Does not matter which of the two the wires goes to which pole.
Solder ends to your phone plug.
