When would you need a new regulator piston?

just curious? Any othe reasons besides leaking out the back when velocity is below 300?

actually, if it is leaking out the back, replacing the O ring on the piston usually fixes. I actually have not seen it needing replacement. (i've seen really old abused mags, and most of them just needs complete O ring replacement to set them right) with oiling them ofcourse.
