His excuse that the tap didn't go deep enough sounds like BS. I am not certain how these tanks are manufactured but I see no reason why they would be tapped for threads after the metal part is already in the tank. This would leave a huge amount of trash (metal filings, cutting oil) inside the tank that would be nearly impossible to remove.
It seems far more likely that the inserts are threaded first, and then put into the tank after being inspected. I find it highly unlikely that something resembling that would ever intentionally make its way into a tank.
