Is this a Lvl 5?

I Had an old mag come through the shop a week or two ago. The fella had a R/T valve to put in, and left the shop the old valve and bolt. I know a LvL 10, I know a ANS bolt, I know a lvl 7 bolt. But I have never seen something like this.
Is it a 7 foamie?

Yes. AGD Level 7 bolts came in foamie and non-foamie versions.
