Last I checked Mike talked quite openly about his inspiration about the characters. They indeed are based on real people.

Butt-head even was inspired by some dumb kid at a concert, something happened (I don't know? heard this story long time ago) the kid turned around with that upper lip thing and told mike "This sucks" and walked away. So voila, instant birth of butt-head.

The reality of that image is that its a mugshot .... apparently the dude broke into a house and burglerized it.... while wearing the grandmothers dress They think the cuts came from a nearby brior patch... weird stuff.
