If you are being attacked while you are logging out, but you dont die, but ur still currently being attacked will you ever wake up in the hospital?

Also, is there a certain time limit that you have to go train up to the next level?

No. No time limit.

You can actually train up as slow as you want, and you can even cancel your account and come back months later and it will be as you left it once you re-register. I actually stopped playing for roughly 2 1/2 months. Came back and resetup on their site and it was like normal. They dont delete characters.

As for being attacked. I dont know. I would assume you'll appear just where you were when you logged out (since the server simply captures you're placement on the map). BUT... I'm not sure since normally when I get in a jam like that its enough baddies that I cant last 30 seconds of no attacks to log out.
